Hospital to home

Practical support after a hospital discharge, helping the person recover safely and confidently at home.

Returning home from hospital can feel difficult, especially when medication, mobility or daily routines have changed. We can help prepare the home, arrange essential shopping and make sure practical support is ready.

We cover that gap. Often it is short — two or three weeks — and then it stops, because the point is to help the person recover, rebuild confidence and become as independent as possible rather than to start a permanent package.

Who this suits

When it is not the right answer. We are not a reablement therapy service. If someone needs physiotherapy or occupational therapy input, that comes through the hospital or the community team — we support the plan, we do not replace it.

What is included

What hospital to home covers

The care plan is written with you, so this is a starting point rather than a fixed list.

  • A home that is ready

    Preparing the home, with permission, so it is warm, comfortable and ready for the person’s return — heating on, bed made, and something in the fridge.

  • The new medication

    Medication support during the changeover period, where agreed following an assessment and included in the care plan. All medication support is recorded appropriately.

  • Checking the home

    We identify and report possible hazards in the home and follow any guidance provided by the hospital, physiotherapist or occupational therapist.

  • Personal care while you recover

    Washing and dressing help for as long as it takes, and no longer.

  • Meals and fluids

    We prepare suitable meals and encourage regular drinks, following any dietary or professional guidance.

  • Follow-up appointments

    Getting you there, and making sure what was said gets written down.

How quickly can you start?

We may be able to begin within 24 to 48 hours, subject to a safe assessment and suitable carers being available. Ring us as soon as a discharge date is being discussed rather than on the day.

How does the hospital referral process work?

The hospital team can contact us directly. We then complete an assessment and agree a safe discharge care plan.

Can families contact you directly?

Yes. The person or their family can contact us without a professional referral.

What information is needed before discharge?

We need details about the person’s health, medication, mobility, daily routine and support needs.

Can you help with medication?

Yes. Medication support is provided following an assessment and recorded in the care plan.

Can you provide personal care and meal support?

Yes. We can help with washing, dressing, continence, meals and drinks during recovery.

Can you provide overnight care?

Yes. Sleeping-night or waking-night support may be arranged following an assessment.

Can you support mobility and moving and handling?

Yes. Carers follow the agreed moving-and-handling plan and use suitable equipment.

What happens if the person becomes unwell again?

We follow the care plan, contact the appropriate healthcare professional and inform the family. In an emergency, we call 999.

How will the family receive updates?

With the person’s consent, we provide agreed updates and report any important changes promptly.

Can it be short term?

Yes, and often it should be. We review the care with the person, their family and relevant professionals and agree when support can be reduced or ended.

Can support continue if longer-term care is needed?

Yes. We can review the care plan and arrange ongoing support where appropriate.
