Most people, asked where they would want to be, say home. Making that possible takes careful planning and a team that works closely with the person, their family and healthcare professionals.
We are the hands-on care in the house: personal care, comfort, presence, and support for the family who are living through it. The clinical side stays with the district nurses, the GP and the hospice — and we make sure everyone is talking to each other.
When it is not the right answer. We do not give injections or manage syringe drivers — that is district nursing. What we do is make sure someone is there in between, which is usually the gap families feel most.
What is included
The care plan is written with you, so this is a starting point rather than a fixed list.
Gentle washing, mouth care, repositioning, skin-care support and pressure-area monitoring, following the agreed care plan and professional guidance, at whatever pace the day allows.
Sitting with someone so they are not alone, and so the family can sleep, eat, or go outside for ten minutes.
We record what we see and pass it on, so the district nursing team are not working blind between visits.
Practical help with the house and the day, and clear updates about the care being provided, with any changes or concerns reported promptly to the family and healthcare team.
Waking or sleeping nights so the family are not doing every night themselves.
Following a death, we will treat the person and their family with dignity and compassion, follow the agreed procedures and provide appropriate practical support during this difficult time.

District nurses, GPs and hospice teams manage injections, syringe drivers and changes to medicines. Our carers provide agreed medication support and report any concerns.
We follow the person’s care plan, contact the appropriate healthcare professional and inform the family. In an emergency, we call 999.
Yes. With the person’s consent, we work closely with everyone involved in their care.
Yes. We can arrange waking or sleeping-night care following an assessment of the person’s needs.
Yes. Care can begin once we have completed a safe assessment and suitable carers are available.
We listen to the person and respect their choices and advance-care wishes. Where needed, we follow the Mental Capacity Act and best-interest process.
We can provide personal care, comfort, companionship and overnight support, giving family members time to rest.
We follow the agreed plan, contact the appropriate healthcare professional and family, and protect the person’s dignity throughout.
We give urgent palliative-care enquiries priority and will explain honestly how quickly suitable support can begin.
